MDM Transparency
See exactly why a visit earns its E/M level.
CognIQ scores medical decision making from the finished note the way the 2026 AMA E/M guidelines do — the complexity of the problems addressed, the data reviewed, and the risk of management. No black box: every level is on screen before you bill.
What It Does
Three elements at a glance
Complexity, Data and Risk each get their own level — straightforward, low, moderate or high — so you can see which elements carry the visit.
Right where you code
The Medical Decision Making box sits at the top of the visit’s Procedures tab, above the CPT codes you’re reviewing.
Built from your documentation
MDM is calculated from the SOAP note itself. Edit the note and regenerate MDM, and the levels follow what the chart now says.
How It Works
Finish the visit and review the SOAP note.
Tap Approve codes — CognIQ calculates MDM from the note alongside the codes.
Open the visit in the Coding tab: the Medical Decision Making box shows each element’s level.
Changed the note? Regenerate MDM to recalculate.
