Notes in the shape your practice already uses.
CognIQ loads your practice’s note templates alongside the ones you create. Apply one to a finished note — a whole-note format like your annual physical, or a single section like normal vitals — and CognIQ fills it in from this visit.
What It Does
Your practice’s templates
Templates set up for your practice are there for every doctor from day one, next to your own.
Easy to find
Search by name or browse by category — Problem-Specific, Vitals, Physical Exam, History, Review of Systems, Assessment & Plan and Procedures. Each card shows which note sections it fills.
Whole note or one section
A whole-note template rewrites the note into its format with this patient’s findings. A section template, like Normal Vitals or Normal Exam, fills just the section it belongs to.
Make your own
Create custom templates with your own name, category and sections. Fill a template’s fields by typing, or speak them all at once.
How It Works
Record the visit — CognIQ writes it as a new-patient or follow-up note, based on the patient’s history.
On the finished note, open Templates.
Search or browse by category, then tap Apply.
Need one that doesn’t exist yet? Tap Create custom template.
